Registering as a New Patient

Registration can be in person at the Practice where both an NHS and Practice registration form need to be completed, or you can register online through either our registration QR code or the button below. 

Patients should note that it will be necessary to provide proof of identity and current address if registering in person.

You must be residing at a permanent address within the surgery’s catchment area to be eligible for registration. The map tool below allows you to check whether you are in the catchment area by entering your postcode.         

On successful registration, patients are allocated a named accountable GP who will take lead responsibility for the coordination and successful delivery of your care at the practice. For patients aged 75 or over, this GP will work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient.

If you wish to find out the name of your accountable GP please contact the surgery. If you have a preference as to which GP this is, the practice will make reasonable efforts to accommodate this request.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
