Registration must be in person at the Practice where both an NHS and Practice registration form need to be completed. Patients should note that it will be necessary to provide proof of identity and current address.
You must be residing at a permanent address within the surgery's catchment area to be eligible for registration. The map tool below allows you to check whether you are in the catchment area by entering your postcode.
On successful registration, patients are allocated a named accountable GP who will take lead responsibility for the coordination and successful delivery of your care at the practice. For patients aged 75 or over, this GP will work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient.
If you wish to find out the name of your accountable GP, please contact the surgery. If you have a preference as to which GP this is, the practice will make reasonable efforts to accommodate this request.
