Registration must be in person at the Practice where a registration form is to be completed. Patients should note that it may be necessary to provide proof of identity and address.
We work as a group Practice. Registrations will be nominally with one Doctor. We are happy for you to see the Doctor of your choice and encourage patients to see the same Doctor as much as possible to promote continuity of care.
